Final Analysis:

Because of Tofu's lack of real presence in most of the series, he has to be taken essentially as a "as you see him" kind of character. He has his two sides: competent doctor, and lovesick maniac. He seems cheerful and happy with his lot in life, and very probably is. There's not really much to be seen under the surface with Tofu.

What we can do to fill in the blanks a bit is look at his relationship with his mother. In a story that appears only in the anime, Tofu gets a visit from his mother, who is impatient for grandchildren. Interestingly, although Tofu is a grown man who is used to telling others what to do (in a firm, doctorly way), he is unable to stand up to his mother when she pesters him about getting married, and in his attempts to not deal with the situation he ends up getting roped into fake engagements with both Nabiki and Akane. When his mother forces the two to compete against each other to see who has better hips for childbearing, Tofu is unable to step in and stop the competition (even after Ranma gets dragged into it). The episode ends, however, with Tofu finally stepping in and telling his mother he doesn't want to marry yet, and will do it when the time is right - only after Kasumi makes an introduction. Tofu's mother puts two and two together and realizes that Tofu knows who he plans to marry, but is going to take his own time with it.

Tofu's passivity in this episode echoes his odd relationship with Kasumi. Although he is respected and obeyed by everyone around him (not that he ever abuses his authority, which is probably even more why people respect and obey him), and although his confidence is clearly high when it comes to everyday life, he cannot deal with his mother or with his feelings for Kasumi. He lets his mother push him around and plan his life for him; he can't contain his giddiness when Kasumi is around. Essentially the only time he has managed not to overreact around her is when she is meeting his mother; this is also the only time he has a natural conversation with her, at least that we see.

What we can guess from this is that, when outside of his (admittedly large) box, Tofu just doesn't know what to do. He knows martial arts; he knows pressure points. He doesn't know love or romance or family planning, and in the end he gets a little freaked out by the prospect of any of them.

As I said, if Tofu's presence continued in the rest of the series, I would speculate much more heavily about his motivations. I consider him a character with a lot of past to be revealed, a lot of personality shaping to show off... But none of it ever came, so in the end I have to assume that he's pretty much what we saw here, and not much more. Then again it also gives fanfiction free reign! And if I read fanfiction I'd be thrilled by that. ;)
